header row for format-opts

It would be nice to be able to get format-opts to print a header row with labels for the different columns.

Perhaps it's possible to generalize this, e.g. to get back the coordinates from format-opts so you can generate a header string yourself (or anything after the formatted options).

I thought of doing something like this at first myself. The problem is, that if the header fields are wider than any of the columns in format-opts, the header should affect the padding in the columns.
But maybe splitting the functionality into a print-table and a spec->help-table could work.
